Header search is the named exception: a visually hidden `<label>`, placeholder “Zoek”, and a submit with text and/or magnifying-glass. Ordinary fields still need a visible label; this exception does not pass elsewhere. Fail if an ordinary field uses placeholder as its only label citing this exception, or if header search lacks a visible Search control and accessible name. Apply only to site header search with a visible Search affordance.

Not for ordinary fields; placeholder-as-only-label remains banned elsewhere.
